SANTA CLARA, Calif. - The Santa Clara University men's tennis team will hold a two-day walk-on tryout the first week of classes for any student interested in joining the team in the fall. Tryouts will be held on Monday and Tuesday, Sept. 22-23 at 2:30 p.m. at the Degheri Tennis Center.
Each student must complete and turn in the following forms on the first day of tryouts:
Student-Athlete Forms
Insurance Declaration
Athlete Physical
Athletic Waiver and Release of Liability
Medical History Questionnaire
Orthopedic/Neurological Exam
Athletic Accidental Medicare Coverage
Authorization for Disclosure of PHI
Notice of Privacy Practice
